Vision Insurance

Vision insurance is offered through Superior Vision. Superior offers a large national network, deep discounts with enhanced benefits.
If you utilize an out-of-network provider, Superior Vision will reimburse you up to the allowed amount shown. You must submit a claim form for reimbursement. Log on to www.superiorvision.com and select the Superior National Network to find a provider near you.

What does the plan cost?

Head Start pays a large share of the premium.
Premium is deducted on pre-tax basis from 1st 2 paychecks of the month.
Pre-tax so you do not pay taxes on the cost of the premium.
